State medical license
Illinois physicians apply to the Illinois Department of Financial and Professional Regulation (IDFPR), Division of Professional Regulation. Illinois is an IMLC member, so SPL-eligible physicians from other Compact states can use the expedited Compact pathway.
Illinois requires graduation from an accredited medical school, postgraduate training, USMLE/COMLEX passage, and Illinois Sexual Harassment Prevention training (one-time).

Controlled-substance registration
Illinois requires a separate state-level Controlled Substances License (CSL) issued by IDFPR, in addition to the federal DEA registration. The Illinois CSL must be obtained before applying for or renewing the federal DEA registration with an Illinois address.

Medicaid enrollment

Illinois Medicaid enrollment is administered by HFS through the IMPACT (Illinois Medicaid Program Advanced Cloud Technology) provider enrollment system. Most beneficiaries are in HealthChoice Illinois managed care plans.
